Subsidised Home Modifications

How does it work? What should I do?

Step One

SITUATION 1: You need to contact My Aged Care (MAC) on telephone number 1800 200 422 of what you would like done to your house, to support your ability to continue to live in your home safely and independently.

SITUATION 2: You might have gone into hospital after an incident and the Occupational Therapist may recommend modifications to your home.

Step Two

SITUATION 1: My Aged Care then contacts a Regional Assessor (RAS) who will come and meet you for an assessment. They will meet you at your home at an agreed time and date. You will be receiving a call from a RAS representative to arrange this. (please discuss time frames with My Aged Care).

SITUATION 2: The hospital Occupational Therapist will visit your home and draw up diagrams and plans detailing the changes needed.

Step Three

In both situations, the Occupational Therapist's diagrams will be forwarded to our service (Home Modification Solutions) for quoting.

Step Four

We will process your referral from the Occupational Therapist and you will be contacted by our office to provide you with a subsidised quote estimate and to discuss a suitable day and estimated time for work to commence. We are also able to organise a written quote if desired. If you are agreeable to the costs, we will proceed to book a trades-person or contractor.

IMPORTANT: If you would like to make any changes to your request or for any unforeseen reason you are not available on the day, please contact our office as soon as possible, on 9524 1100.

Step Five

When the trades person or contractor attends your home and if you agree for them to do the work on the day, please ask for clarification of cost to you before the work commences. Once completed they will ask you to sign the green Work Completion form.

Step Six

When we receive notification that the work is completed, we will send you an invoice for the agreed amount. Payment instructions will be detailed on the invoice.

Step Seven

We hope our work will make a difference in your day to day life at home. In the future, we may contact you to provide us with your feedback and opinion on our performance. This is just to keep us on track by ensuring we are doing all the right things for you and will help better our services for the future.

Subsidised Home Maintenance

How does it work? What should I do?

Step One

TIP: Start to make a list in advance, of the issues you encounter with your day to day living in regards to your safety at home including any modifications and repairs around the house. This way, you will be prepared and ready to discuss them with your Regional Assessor when they visit.

Step Three

Once the RAS representative has completed the assessment, they will then forward a referral to our service. When we receive the referral, we will acknowledge it by sending you a 'welcome pack' which will give you a further understanding of the work we do and our contact details.

Step Four

Please contact us when the need arises. We will process your request and contact you to discuss a suitable day and estimated time for work to commence and/or assess to determine costs and provide you with a subsidised quote. We are also able to organise a written quote if desired. If you are agreeable to the costs we will proceed to book a trade person.
